For medicine, it is absolutely indispensable. In general, I find that the questions are easier than those on the shelf exams, but the explanations are pure gold and helped me to answer shelf questions correctly on a number of occasions. I recommend buying the bank early and doing the appropriate questions during each clerkship.I have taken all of the shelves, step 1, and step 2. For each of my shelf exams and for step 2, the exams focused largely on high yield, essential topics. It is also far from an impossible task to do well on these exams, or at least be well above the failing cutoff. And yes, in some ways med school is a lot harder. I felt the NBMEs were very indicative of the actual shelf. I did have IM before FM, so not sure how much of a difference that made.

I did all of UWorld's medicine questions, and watched Emma Holliday's review 3 times before the test and I had a 76 on the test (just a bit above average I believe). I also made ANKI flash cards. If your goal is an A (high honors or whatever) a bit more than that should be fine. Keep in mind, the medicine shelf covers a crazy amount of material.
