The thermal actuator is used to see if the engine coolant is warm. If it is then it pushes out setting the cam physically. Now unless the thermal actuator is stuck then you may have an issue. When the coolant cools the actuator will retreat back in. When your truck is cold the actuator will be in and you will set the idle higher.
